When it is used on startup, the width gets stretched approximately 1.8 times as much as the height, something to keep in mind when designing an alternate (i.e. make lettering, etc. tall and narrow).
You can create one simply by renaming a 320x400 256-color bitmap to ".sys", but it won't palette-shift. To do that you need to look for third-party utilities.
You're not limited to a scroll bar, either. My favorite was a stormy startup screen that read "Shattered Windows 95 with Bugs!" (instead of Microsoft Windows 95 with Plus!). It had flashing lightning
