What you see is most likely a misconfiguration of the write combine buffers that address the HW acceleration. Looks like you have multiple copies of your video card running as ghosts and one of them occupies the correct base address so it cannot be used by the valid copy.
check your device manager but you need to do it in safe mode, otherwise you cannot see shadowed devices.
Delete all hardware that is present in duplicates, that is, all copies of it. Don't worry, on the next boot-up, Windows will find the missing devices and reinstall them.
